For NIS Alumni Only
This form is intended only for NIS alumni. We will make every effort to complete your request as soon as possible. Expect delays for requests received on the weekends or summer/winter holidays as staff are not working normal business hours.
Please note these basic guidelines for submitting this request form:
- Your request will be fulfilled as soon as possible; postal mailing of official documents can take longer depending on the country to send so please ensure that your request is made far enough in advance.
- If you request documents to be sent to somewhere else, we will also email you to inform you that it has been sent.
- Correctly identify the documents you require; NIS cannot be responsible for errors or omissions in your requests;
- Complete all required fields;
- Failure to follow these guidelines will result in the delay of processing your request.
